Driver Qualification File Checklist
Every document FMCSA requires in a DQ file, in the order auditors expect, with the 49 CFR 391 citation next to each one. Free PDF.
What’s on the checklist
- Driver application with complete employment history per 49 CFR 391.21
- Motor vehicle record (MVR) at hire and annually thereafter
- Road test certificate or equivalent per 391.31 / 391.33
- Medical examiner’s certificate and the National Registry verification
- Previous employer inquiries, including the safety performance history per 391.23
- Drug and alcohol pre-employment test results and consortium documentation
- Annual review of driving record per 391.25
- Annual driver certification of violations per 391.27
What auditors actually open
The DQ file is the second tab the auditor opens. They are looking for three things: every required document is present, dates are within the acceptable windows, and the file is current for active drivers. Pattern findings on missing or expired documents are one of the three issues that fail a new entrant audit.
